Course Registration Details for Cooper Employees

  1. ALL employees must complete a registration form for each course they plan to attend.
  2. Reservations are first come first serve.
  3. Submit Conference Request Form to your Clinical Manager or Supervisor for signature and send to Patient Care Services - Education Division. (The form may also be faxed to (856) 968-8385.
  4. You will receive E-mail notification confirming your registration. Call Patient Care Services - Education Division at (856) 342-2459 if you have not received confirmation.
  5. Students and Faculty affiliate of schools of nursing with Cooper University Hospital may attend courses per seat availability. Conference registration forms must be completed.

Course Registration Details for Non-Cooper Employees

  1. Complete the registration form and send check payable to Patient Care Services - Education Division at Cooper University Hospital , One Cooper Plaza, Camden, New Jersey 08103-1489.
  2. You may fax a copy of your registration form (with the original registration and check in the mail) to hold a space on the attendance sheet until two weeks before the course. Two weeks before the course the space will be given to a paid registrant if we have not received your check.
  3. You will receive E-mail notification, etc – Please bring your confirmation along with you so that Security may direct you to the appropriate location.
  4. The Cooper Health System reserves the right to cancel courses due to insufficient enrollment, in which case registrants will receive a full refund.
  5. In order to receive full or partial refund from The Cooper Health Systems, you will need to send a copy of the front and back of cancelled check.

Accreditation / Designation / Disclosure
Cooper University Hospital is an approved provider of continuing education by the New Jersey State Nurses Association (P 101-5/03-06).

Certification Maintenance Information
Participants enrolled in courses which offer CEUs or contact hours, will receive certificate upon completion of the course. Completion may include successful passing of a written and/or practical examination.

PCS - Education is not responsible for maintaining copies of your certificate. Requests for copies of course certificate will be charged $10.00 processing fee for each.

Faculty
All courses utilize select representatives from the Education Specialist group, Clinical Instructor group, Clinical Nurse Specialist/Nurse Practitioner group, Medical Staff, Hospital Staff, and/or outside Consultants.

Flyers / Announcements
Flyers will be distributed in-house as reminders prior to all courses and e-mailed to those addresses on file. Please verify all course dates and locations by checking unit mailboxes and bulletin boards. There are also (2) bulletin boards in front of the Nursing Administration office/Cafeteria.

Parking
Cooper Staff – Day Staff should park in designated lot. Night Staff (coming to a daytime class) should park in the Medical Arts parking lot located behind the old church near Dorrance entrance (6th & Stevens Street).

Non-Cooper Staff please note parking is not included in the fee for the course. Fee for parking is approximately $6 per day. Non-Cooper staff should park in either One Cooper Plaza (attached to main hospital) or in Three Cooper Plaza Parkade on Haddon Avenue across from main hospital. Considering using transportation via the High-speed Line – it is located across the street from Cooper. The stop is “Transportation Center”.

Meals
Breakfast is included in all course fees. Lunch is on your own.
